Thorough clinical assessment
A focused history: symptom pattern, onset, severity, triggers, medications, family history and lifestyle factors.
Complete physical exam emphasizing abdominal and systemic signs.
Symptom scoring when appropriate (e.g., reflux, IBS, or liver symptom scales).
Targeted laboratory testing
Basic blood work: CBC, liver and pancreatic enzymes, inflammatory markers.
Stool tests: infection panels, occult blood, inflammatory markers (calprotectin) or fat quantification when malabsorption is suspected.
Serology when indicated (e.g., celiac screens, autoimmune markers).
Non-invasive imaging and breath tests
Ultrasound for gallbladder, liver and biliary assessment.
CT/MRI recommendations when cross-sectional imaging is necessary.
Breath tests for small intestinal bacterial overgrowth (SIBO) or Helicobacter pylori where indicated.
Endoscopic assessments
Upper endoscopy (EGD) to visualize the esophagus, stomach and duodenum, take biopsies and treat bleeding or remove small polyps.
Colonoscopy for colorectal screening, polyp detection/removal and evaluation of lower GI bleeding or chronic diarrhea.
Advanced endoscopic procedures (ERCP, EUS) coordinated with specialists when needed.
Physiologic testing and motility studies
Esophageal manometry for swallowing problems and achalasia-like symptoms.
pH or impedance monitoring for reflux that doesn’t respond to therapy.
Anorectal manometry and pelvic floor testing for defecatory disorders.
Multidisciplinary case review
